Sliding Fee Program
A Sliding Fee Discount Program is provided to eligible individuals regardless of their ability to pay.  Qualification will be determined by the amount of household members and the annual household income. All individuals and families with annual incomes at or below 200% of the Federal Poverty guidelines will qualify for a sliding fee discount.  In order for you to receive a reduced fee you must provide proof of income.  Payments for discounted office visits, labs, radiology or ECG is requested at the time of service.

Integrated behavioral health care embraces a “whole-person” view of health and wellness. Thoughts, feelings, actions, and life situations can impact your sense of well-being. We provide this integrated care approach because we understand that your primary care provider can and should be a consistent resource for finding help when you aren’t feeling well—whether physically or emotionally.

Our health care team includes staff who are experts in helping people cope with life’s challenges. We have behavioral health consultants, care coordinators, and others who provide services to assist with stress management; treat depression and anxiety; help with sleep difficulties, chronic illness, or pain issues; and a wide range of other common problems. Integrated care staff can also assist you with finding other community resources and services if needed.
